Appointments
Appointments are scheduled Monday thru Friday during normal business hours. Please call our main number (913) 642-0200 and select option 1, or use the on-line appointment form. We will make every effort to get you in as quickly as possible. In order to expedite the scheduling process please have the following information available when you contact Carondelet Orthopaedic Surgeons, P. C.
New Patients:
- Your full name
- Date of birth
- Home/Work phone numbers
- What body part you need to be seen for
- How you injured that body part
- Is your injury work related
- Insurance coverage (see list of participating insurance companies)
– If you have an HMO plan you need to obtain a referral from your PCP
- If not HMO, were you referred to us by another physician
- Were you seen in the Emergency Room
– If so please bring your x-rays with you
- If MVA/Liability Claim - (see notice regarding our policy)
**Please note:
If we are seeing you for back or neck problems, please arrive 30 minutes prior to your appointment time to fill out the necessary paperwork. All others please arrive 15 minutes prior.
Existing Patients
- When were you last seen
– If longer than 6 months, we will ask you to update your registration and billing information
- Is this a new injury or follow up for an existing problem
